Job description
A fantastic opportunity is available for an experienced Practice Nurse to join a busy GP practice in Manchester on a locum basis. This role offers flexibility, with 2–3 days per week and working hours tailored to suit your availability. The successful candidate will be confident delivering a broad range of practice nursing duties, including Travel Risk Assessments (TRD) and Chronic Disease Management (CDM), while working within a supportive primary care environment.
Key Responsibilities
• Deliver routine practice nursing services within a GP setting
• Manage and support patients with chronic diseases (CDM)
• Conduct Travel Risk Assessments (TRD)
• Provide high-quality patient care in line with practice protocols
• Maintain accurate patient records using EMIS Web
Requirements
• Previous experience working as a Practice Nurse within primary care
• Confident with all TRD and CDM duties
• Experience using EMIS Web
• Strong communication and organisational skills
• Ability to work independently and as part of a multidisciplinary team
